| How healthcare is paid for | The NHS: tax-funded and free at the point of use. The NHS Constitution for England puts it as a right -- care is provided "free of charge, apart from certain limited exceptions sanctioned by Parliament" -- and "you have the right to access NHS services. You will not be refused access on unreasonable grounds." But health is DEVOLVED: England, Scotland, Wales and Northern Ireland run four separate systems with different charges, different waiting-time rules and different fertility policies, and a UK-wide average is usually a fiction. Migrants pay for access up front through the Immigration Health Surcharge, charged with the visa application: GBP 1,035 per year for most applicants and GBP 776 per year for students, their dependants, Youth Mobility Scheme holders and under-18s -- GBP 3,105 for a three-year work visa, per person, before you have used the NHS once. | IVF cycles the state covers | NICE-based eligibility is 3 full cycles for women aged 39 or under and 1 full cycle at 40 to 42 -- "if you're aged 39 or under, you may be eligible for 3 full cycles of IVF on the NHS" after two years of trying or 12 cycles of artificial insemination including at least 6 IUI; "if you're aged 40 to 42, you may be eligible for 1 full cycle" on the same criteria plus never having had IVF before. Above 42 there is no NHS route. BUT THE GUIDANCE IS NOT THE ENTITLEMENT. "Local integrated care boards (ICBs) assess who is able to have IVF in their area and make the final decision about funding treatment", and they may be stricter than NICE on age, BMI, smoking and -- the one that catches step-families hardest -- "you do not have any children already", including a child from a previous relationship. In practice most English ICBs fund one cycle rather than three. Scotland and Wales set their own policies. Two couples with identical medical facts, one in each of two neighbouring English ICBs, can be offered three cycles and one cycle respectively.
